Abstract:

Permanent record cards for registered and practical nurses including married and maiden name, address, birth date, school of nursing attended and graduation date, certificate and license numbers, examination and registration dates, and specialization and renewal information. Because applications 2-4154 are missing from the main registration records in Series 2676, this file contains the only known biographical information on the earliest nurses to be registered in Wisconsin. The majority of the oldest licenses may be found on Reel 9.
