Abstract:
Reports of physical and psychiatric examinations of inmates of penal and reformatory institutions administered by the Psychiatric Field Service at the time of admission, application for parole or pardon, and for special causes. Psychiatric examination reports include name, age, family history, social history, military history, history of offense, institutional history, inmate's future plans, I.Q., results of a “psychoneurotic inventory” test, and a narrative assessment of the individual by the examiner. Physical examination reports include inmate's physical characteristics, medical history, present complaints, physical abnormalities and laboratory results. The files also contain copies of correspondence between division offices and state institutions, court orders and other documents regarding the status or condition of inmates. Includes records of men, women and boys age 16 and older incarcerated at the Wisconsin Home for Women, Wisconsin State Prison and Wisconsin State Reformatory.
