Cards (CY-22 and FS-SS-22) showing children under commitment or receiving services from the Division of Community Services' direct services units in regional and district offices. Information includes agency code, commitment or service number, acceptance and/or legal transfer date, child's name, alias and birthday, county of commitment and legal settlement, reason, type and level of casework, sex, heritage, birth and legal status, and religion of child, natural parent(s) name, address, and marital status, child's type of living arrangement and name and address of placement(s), maintenance source and amount, other agencies active, and caseworker name. The file includes Social Data Change Sheets (CY22a and FS-SS-22a) which update all information above.
